Performance Membership Change Request

Fill out the form to modify your monthly Performance Membership. You can choose to pause, restart or cancel.

To ensure your package change is effective from the start of the next month, please submit your request through this form no later than 11:59pm on the 20th of the current month. 

For instance, if you submit your request on July 13, the changes will be implemented starting August 1st. However, if the request is submitted on July 28, the changes will come into effect from September 1st.
